Interior of a crappy motel room, Cas lying in bed, half-wearing a robe.
Cas: "I miss him... Why would he just disappear?"
Sam [on phone]: "Who says he had a choice?"
Cas: "Well then, who wrote the note? If there's any chance... any chance at all that Dean is still..."
Sam: "Still... even remotely Dean?"
Cas sadly hangs up the phone, as does Sam. The somber music gives way to the sounds of Right Said Fred's "I'm Too Sexy," and we then cut to Dean in a bar.
Dean [singing karaoke]: "I'm too sexy for my love; too sexy for my love; love's going to leave me." 10.01
Sam: "What the hell are we doing to him, Cas? I mean, even after I gave him all that blood, he still said he didn't want to be cured, that he didn't want to be human."
Cas: "Well... I see his point. You know, only humans can feel real joy, but... also such profound pain. This is easier."
[Dean wakes up, the black of his eyes dissipates. He looks up to Sam and Cas, groaning and letting out a breath.]
Dean: "You look worried, fellas." 10.03
Up until this episode, Dean thought Cas was dead. In 9.22 he learned that Castiel was dying, why he chose to take on a suicide mission against Metatron. And in 9.23 Metatron told him that he had Castiel imprisoned and would torture him to death. So Dean died thinking Cas would soon follow. The profound pain he was running from? Was this. Castiel's death. (If you don't understand why dealing with Castiel's death was something that Dean just could not go through again, watch S07.) That's why the cure didn't work until Castiel. That's why Castiel didn't need to do anything beyond just being there. Because he was the cure. Castiel is the cure, because the disease is the fear of not having Castiel in his life. Remeber 9.03 where Castiel's mirror Maurice was an anagram for "I am cure". That's how long this has been coming.
Cas: "You look terrible."
Dean laughs: "You know, it wouldn't kill you to lie every now and again."
Cas: "No, it wouldn't kill me. I just... You..."
Dean: "Forget it. Well, you, on the other hand, you... looking good." 10.03
It has only been a fucking moment since Dean became human again and the first thing he does is flirt with his boyfriend.
- Ketch: "You don't look good." Dean: "Yeah well, you're not my type either." 13.18 It's easy to see who is Dean's type.
Dean: "So... Are you back?"
Cas: "At least temporarily. It's a long story. Crowley, stolen grace. There's a female outside in the car." [Dean stares at him, and Cas shakes his head.] "Another time."
Dean: "Well, thank you for, uhm... stepping in when you did. What did Sam say? Does he want a divorce?"
Cas: "I'm sure Sam knows that whatever you said or what you did, it wasn't really you. It certainly wasn't all you."
Dean: "I tried to kill him, Cas."
Cas: "Dean. You two have been through so much. Look, you're brothers. It'd take a lot more than trying to kill Sam with a hammer to make him want to walk away."
Dean: "You realize how screwed up our lives are that that even makes sense? I'm glad you're here, man."
Cas [starts to leave, then turns back]: "Hey, maybe you should... take some time before you get back to work. Allow yourself to heal. It's... I don't know. The timing might be right." 10.03
Dean sees Siobhan and Kristen, still with their Dean and Cas costumes, kissing.
Dean: "What are they doing?"
Marie: "Hum... Kids, these days, call it hugging."
Dean: "Is that in the show?"
Marie: "Oh, no. Siobhan and Kristen are a couple in real life. Although, we do explore the nature of Destiel in act two."
Dean: "Sorry, what ?"
Marie: "Oh, it's just subtext! But, then again, you know, you can't spell subtext without... s-e-x." [Dean lets the words sink in.]
After that, Dean comes back to his brother outside. He just explained everything about the Destiel part.
Sam: "I don't understand."
Dean: "Me neither."
Sam: "I mean, shouldn't it be... Deastiel?"
Dean: "Really? That's your issue with this?"
Sam smirks: "No, of course it's not an issue. You know... How about Sastiel? Samstiel?"
Dean [now upset]: "Ok, alright. You know what? You're gonna do that thing, where you just shut the hell up. Forever."
Sam [laughs, then looks at Dean]: "Casdean?"
Dean: "Shut your face! Get in the car." 10.05

Hannah [standing naked in front of Cas]: "Does this bother you?"
Cas: "Bother? I'm not..."

This is why "You're my brother, Cas" line is a total bullshit, Dean Winchester. Because if you loved Cas as a brother, you would say something like "my family", "my brothers", "my best friends", just to establish the difference, but no, you pulled out "people who love me". Because you know he loves you differently than Sam and you know you love him differently too, you emotionally ignorant idiot.
Dean: "Cas, listen to me. There's some stuff you just got to let go. Okay? The people you let down, the ones you can't save... You got to forget about them. For your own good."
Cas: "Is that what you do?"
Dean: "That's the opposite of what I do. But I ain't exactly a role model."
Cas: "That's not true. (...) How are you, Dean?"
Dean: "Fine. I'm great."
Cas: "No, you're not. (...) Is the Mark of Cain still affecting you? Dean? Dean?"
Dean: "Cas, I need you to promise me something."
Cas: "Of course."
Dean: "If I do go darkside, you gotta take me out."
Cas: "What do you mean?"
Dean: "Knife me, smite me, throw me into the freakin' sun. Whatever. And don't let Sam get in the way, because he'll try. I can't go down that road again, man. I can't be that thing again." 10.09
Dean entrusts his death to Castiel. (Cas: "I was your salvation, and you ask me to be your damnation?")
Dean once said the reason he couldn't kill himself is because the mark wanted him alive. That means... Dean already tried. He wouldn't put that burden on Cas, if he didn't try it himself.
Claire: "Dean Winchester is a monster."
Cas: "It's possible there's a little monster in all of us."
Claire: "The fact that you'd try to defend him just proves to me that I have to be on my own." 10.10
"Why don't you do us all a great big favor and pick a bloody side!" Crowley to Dean
And Dean chooses Cas, he gives the first blade not to Crowley but to Castiel.
"When your pet angel found my burial site, I thought about ending him and swatting him like a fly. But then I thought about you. Your biggest weakness, the thing I noticed the moment I met you." Cain to Dean 10.12
"Then you'd kill the angel, Castiel. Now that one... that, I suspect, would hurt... something awful. (...) Has it never occurred to you that you're living my life in reverse?" Cain to Dean 10.14
Cain just said that Dean was going to experience what he did, but only backwards. Cain had to kill his brother, then Colette, then the knights of Hell. Dean was told he was going to kill Crowley, then Castiel, then Sam. Colette was the love of Cain's life. What does that say for Dean and Cas? Castiel is Dean's Colette.
Crowley: "How's that grace of yours holding up?"
Cas: "It's fine, Crowley, I'm fine."
Crowley: "Sure. The clock's ticking again, isn't it? Running errands all over the U.S. burning through that rather finite supply of grace... all in a desperate effort to save your boyfriend." deleted scene 10.14
Dean: "What if I said I... I didn't want to die... yet, you know, that I wasn't ready?"
Father Delaney: "Are you expecting to?"
Dean: "Always. You know, the life I live, the work I do... I pretty much just figured that that was all there was to me, you know? Tear around and jam the key in the ignition and haul ass until I ran out of gas. I guess I just thought sooner or later, I'd go out the same way that I live – pedal to the metal, and that would be it."
Father Delaney: "But now?"
Dean: "Now, um... recent events, uh... make me think I might be closer to that than I really thought. And... I don't know. I mean, you know, there's - there's things... there's... people, feelings that I... I... I want to experience differently than I have before. Or maybe even for the first time."
Father Delaney: "Go a little deeper, perhaps..."
Dean: "Yeah. Yeah, I'm just starting to think that... maybe there's more to it all than I thought." 10.16
Cas: "Dean has given up."
Bobby: "And you idjits haven't?"
Cas: "Would you?"
Bobby: "Hell no." 10.17
Cas: "You killed my friend."
Metatron: "Dean is fine, mostly. Can't you get past that?"
Cas: "Never." 10.18
Castiel can't forgive Metatron, not because of what he did to him, not because he ripped out his grace, not because he let the angels fall, but because he took the one thing away from him that mattered the most. Castiel can never forgive because he killed Dean.
- "I've tried. I've tried to see myself as you did. But I know who I am and what I am. I know you watch over me still, but I need you to look away now." Cain to Colette
Dean: "I took down a monster. Because that's what I do. And I'll continue to do that until..."
Cas: "Until you become the monster."
Dean: "You can leave now, Cas."
Cas: "No. I can't. Because I'm your friend."
Dean: "Really? Well let me ask you something. Do you screw over all your friends?"
Cas: "Sam and I were trying to cure you! We still are! We can read the Book now."
Dean: "Oh so what? So you might find a spell that might take this crap off my arm? But even if you do, what's it gonna cost? 'Cause magic like that does not come free. No, it comes with a price that you pay in blood. So thanks, but I'm good." [Dean turns to leave, Cas grabs his shoulder to stop him.]
Cas: "No! You're not. Maybe you could fight the Mark for years. Maybe centuries, like Cain did. But you cannot fight it forever. And when you finally turn, and you will turn... Sam, and everyone you know, everyone you love... they could be long dead. Everyone except me. I'm the one who will have to watch you murder the world. So if there's even a small chance that we can save you, I won't let you walk out of this room." 10.22

Cas tells Dean with no hesitation that, even after Sam and everyone else Dean loves is gone, he will still be there and he actually has tears in his eyes. He basically just said that he'll stay with him forever.
You can red between the lines, Cas has thought about this... a lot. He has made a decision that he is never, I repeat, never going to leave Dean's side. He has contemplated it, planned it, and firmly resolved that this man standing in front of him, covered in blood and reeking of Lucifer's curse, is his future by his own choosing. It has been years since heaven gave him any orders regarding the Winchesters, but Castiel on his own dicided that come what may he will stay with Dean. And that might mean centuries of trying to stop him from killing his way through Cain's bloodline, but Castiel is absolutely willing to do this.
He could go live out his says in heaven, enjoying the eternal Tuesday afternoon oft he kite flying man and leave Dean to this mess, because the Mark of Cain has nothing to do with Cas. This is not his mess to clean up. But instead Cas declares to Dean that he has thought about and made the decision that Dean is his mission for... well pretty much forever since at this point they are both immortal.
Also Cas expects to be the one that Dean doesn't kill. That Dean won't murder him when the mark takes complete control. He said he would watch him murder the world and he'd still be there alive and by his side. Dean knows how to kill Cas but Cas is so sure that Demon!Dean won't do it. How can he be so sure especially after the last episodes ending?
It has been confirmed that Misha and Jensen changed the script ending of 10.22. Originally the writers wanted Castiel to fight back, but Misha states that Cas will never hurt Dean. If either one of us is bound to get hurt from this, I'll do everything in my power to make sure that it won't be you.
Cas: "Dean. I don't want to have to hurt you."
Dean: "I don't think that's going to be a problem."
Dean wants Cas to keep his promise to kill him.
Cas [as Dean wants to go]: "Dean. Stop." 10.22
- Cain: "I did horrible things - for centuries. Bringer of chaos and darkness." Dean: "Then you met Colette..." Cain: "She knew who I was... and what I was. She loved me unconditionally. She forgave me. She only asked for one thing. To stop." 9.11
Rowena: "Let's not call it love."
Cas: "I don't believe you. Everyone loves something." 10.23
"Looks like you have an admirer." Bartender to Dean about Cas in Dean's dream. Then Dean sees Cas beaten up. deleted dream scene 10.23
Rowena: "My son, aside from being a noxious blowhard, suffers from the same malady as you, I'm afraid. Winchester-Derangement-Syndrome."
Cas: "Is that so? Well, I feel fine."
Rowena: "Please, an angel of the Lord, the king of Hell, more power than I could possibly dream of, utterly wasted, shattered at the altar of Winchester. I may be the one in chains, but the two of you are just as much a prisoner as I am. It saddens me. No. It totally sickens me to see so much power go unspent, so much glory unclaimed."
Cas: "Glory? Oh, I have seen glory. And I've seen defeat, but I have seen the glory. I have won epic battles and have reaped vast rewards. And none of it has meant as much to me as the relationships that I formed on earth. Dean... and Sam, they are like a family to me. Nothing, no - no angelic crusade or victor's bounty, none of that could ever hold a candle to that."
Rowena: "You can't honestly think you're happier now. Broken wings, suck up."
Cas: "All I know is that for the first time in a long time... I feel at peace with who I am. Whatever that may be." deleted scene 10.23
